Western Australian certificates are available in person from the Registry Office on application and payment of the relevant fee. If the birth, death, marriage or change of name occurred in another State, contact the relevant Registry Office.
Uncertified copies of birth, death and marriage records are available on application to the Perth Registry only.
Certificates will only be issued where:
When applying for a certificate in person three forms of evidence-of-identity must be provided. Photocopies must be certified by a qualified person.

Photocopies of evidence-of-identity documents will only be accepted if they are certified by a qualified person as being "true copies" of the original documents. Information about who can certify documents is provided on the application form.
Print and complete the relevant application and post it to the Registry with photocopied and certified identification and payment.
