Apply by email

The Registry is currently accepting the following by email:

  • Applications for Certificates of Births, Deaths, Marriages or Changes of Name which were previously registered in Western Australia.
  • Applications to correct or amend a certificate.
  • Applications to Register a Change of Name.

These can be lodged by emailing to

When you send your request please:

  • complete and sign the application form
  • provide copies of the relevant Proof of Identity documents
  • attach any other supporting documentation
  • complete the payment details section.

If you would prefer not to provide credit card details, you will need to submit your application and cheque or money order via the post to our PO Box.

Once approved, certificates will be posted to the supplied address.

Note: Birth Registration forms cannot be submitted by email.  These must be posted to our PO Box.
